Job summary

Franklin VNA & Hospice in New Hampshire is seeking a Licensed Nursing Assistant to provide basic patient care under RN supervision. You will take vital signs, assist with daily activities, and perform light Homemaker duties as needed.

Applicants must have a high school diploma or equivalent, NH LNA certification, and at least 6 months of related experience. The role offers daytime hours, benefits, mileage reimbursement, and paid training.

Qualifications

  • High School diploma or equivalent and graduation from an accredited Licensed Nursing Assist program.
  • Current Certification/Licensure for State of New Hampshire is required.
  • Minimum of 6 months of experience in an LNA or related role required.

Responsibilities

  • Provide basic patient care under direction of the RN and Clinical & Staff Development Manager.
  • Take and record vital statistics, assist with daily living activities, and change linens.
  • Report changes in patient condition to Nursing supervisor and document accordingly.
  • Provide physical support to assist patients with daily activities and mobility.
  • Perform light Homemaker duties and follow safety guidelines.
  • Turn or reposition bedridden patients and monitor health information.
  • Review dietary restrictions and ensure appropriate meals; document intake as needed.
  • Travel throughout central New Hampshire as required.

Provide basic patient care under direction of the RN and Clinical & Staff Development Manager. Perform duties such as taking and recording vital statistics, feed, bathe, dress, groom, move patients and change linens. May also perform light Homemaker duties.

Essential Functions And Responsibilities

  • Adhere to patient/client LNA care plan as set up the while maintaining a healthy and safe environment.
  • Reports and documents patient/client’s needs and responses to the Nursing supervisor.
  • Reports patient/client’s change in condition or situations immediately.
  • Provide physical support to assist patients to perform daily living activities, such as getting out of bed, bathing, dressing, using the toilet, standing, walking, or exercising.
  • Turn or reposition bedridden patients.
  • Record vital statistics or other health information.
  • Review patients' dietary restrictions, food allergies, and preferences to ensure patient receives appropriate diet.
  • Measure and record food and liquid intake or urinary and fecal output, reporting changes to medical or nursing staff.
  • Monitor patients to detect health problems and advise Nursing supervisor.
  • Perform light housekeeping and Homemaker duties deemed acceptable by the agency in conjunction with or in the absence of the agency Homemaker.
  • Perform other duties assigned.
Requirements
Education and Experience
  • High School diploma or equivalent and graduation from an accredited Licensed Nursing Assist program.
  • Current Certification/Licensure for State of New Hampshire is required.
  • Minimum of 6 months of experience in an LNA or related role required.

  • Ability to wear protective clothing such as mask, glove, head cover, lead apron, safety glasses, safety shoes, etc.
  • Ability to travel required. Travel throughout central New Hampshire is required.
  • Current drivers’ license is required.
  • Proof of automobile insurance is required.
  • Ability to pass a motor vehicle background screening is required.
Physical Demands/Work Environment
  • Job requires physical activities including but not limited to; standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, crawling, fingering, grasping, feeling, talking and hearing.
  • Job requires moderate physical work including the ability to exert up to 50 lbs. of force occasionally, 20 lbs. regularly.
  • Job requires close visual acuity including the ability to see at or within arm’s reach.
  • Job is subject to inside environmental conditions; low noise level.
