Heritage Skilled Nursing and Therapy in Oklahoma is seeking a licensed nurse (RN or LPN) to join a supportive, well-structured team focused on high-quality resident care. You will administer medications, monitor residents, and collaborate with physicians.

Qualifications

• Current Oklahoma LPN or RN license.
• Minimum 1 year of experience in skilled nursing or long-term care.
• Ability to administer medications and treatments per nursing standards.

Responsabilités

• Obtain and administer prescribed medications and treatments per nursing protocols.
• Observe residents and report changes to family and physician.
• Take vital signs and assess residents' condition.
• Adjust care plans with changes in condition.
• Provide wound care and collect samples as needed.
• Prepare injections and monitor catheters.
• Maintain documentation and communicate with the care team.

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques.
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Required Experience and Education

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN) License OR Oklahoma Registered Nurse (RN) License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
