Pine Haven Christian Home in Wisconsin is seeking RNs/LPNs for full- and part-time roles that value clinical expertise, compassion, and building meaningful relationships with residents. You will lead with confidence, connect with residents, and thrive in a supportive, team-focused environment.

Responsibilities include guiding nursing assistants, administering medications, documenting in EMR, coordinating with physicians, and upholding nursing standards.

Qualifications

• Current, active license to practice as an LPN or RN in Wisconsin.
• Regular attendance and punctuality to support positive working relationships and quality resident care.
• Ability to work in a fast-paced environment.
• Attention-to-detail and organizational skills.
• Strong teamwork and communication skills.
• Ability to lift up to 40 pounds.

Responsibilities

• Direct the day-to-day functions of the nursing assistant.
• Receive physician's orders and execute as appropriate.
• Chart nurses' notes in EMR reflecting the care provided to each resident.
• Ensure documentation and communication of changes in a resident's needs or condition.
• Prepare and administer medications as ordered by the physician.
• Arrange for diagnostic and therapeutic services as ordered by the physician.
• Consult with the resident's physician regarding care and treatment.
• Review the resident's EMR for treatments, orders, diets, etc.
• Maintain established nursing standards.
• Check that prescribed treatments are properly administered by CNAs and assess resident status.
• Notify physician and POA/Guardian of changes in condition.
• Administer professional services such as catheterization, tube feedings, wound care, etc.
• Ensure care is provided per the care plan and resident wishes.

Full And Part Time Positions Are Available

AM (6:00am-2:30pm) PM (2:00pm-10:30pm) NOC’s (10:00pm-6:30am)

All scheduled positions require an every other weekend and every other holiday rotation.

• Direct the day-today functions of the nursing assistant
• Receive physician's orders and execute as appropriate
• Chart nurses' notes in an informative and descriptive manner that reflects the care provided to each resident in EMR
• Ensure appropriate documentation and communication of changes in a resident's needs and/or condition
• Prepare and administer medications as ordered by the physician
• Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician
• Consult with the resident's physician in providing the resident's care and treatment
• Review the resident's EMR for specific treatments, medication orders, diets, etc.
• Maintain established nursing standards
• Make periodic checks to ensure that prescribed treatments are being properly administered by certified nursing assistants and to evaluate the resident's physical and emotional status
• Notify the resident's physician and P.O.A./Guardian of changes in condition
• Administer professional services such as; catheterization, tube feedings, wound care, etc.
• Ensure that personnel providing direct care to residents are providing such care in accordance with the resident's care plan and wishes

Requirements

• Possess a current, active license to practice as an LPN or RN in the State of WI
• Understands and supports Pine Haven's Mission, Vision and Values and Resident-Centered Engagement Principles
• Regular attendance, including timely arrival at work, to support positive working relationships with fellow team members and to support quality resident care
• Ability to work in a fast paced rapidly changing environment
• Attention-to-detail and organizational skills required

• Ability to lift up to 40 pounds
