Job summary

Pine Haven Christian Home in Sheboygan, WI is seeking compassionate nurses (LPN/RN) to provide resident-centered care in a team-focused, supportive environment. Both full-time and part-time roles are available with varied shifts, including days, evenings, and nights.

Responsibilities include administering medications, charting in EMR, coordinating with physicians, and ensuring care plans are followed. Candidates must be licensed in WI and able to lift up to 40 pounds.

Qualifications

• Possess a current, active license to practice as an LPN or RN in the State of WI.
• Ability to lift up to 40 pounds.
• Strong teamwork and communication skills are critical.
• Regular attendance, including timely arrival at work.
• Attention-to-detail and organizational skills required.
• Ability to work in a fast paced rapidly changing environment.

Responsibilities

• Direct the day-today functions of the nursing assistant.
• Receive physician's orders and execute as appropriate.
• Chart nurses' notes in EMR.
• Ensure documentation and communication of changes in resident's needs.
• Prepare and administer medications as ordered.
• Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician.
• Consult with the resident's physician in providing the resident's care and treatment.
• Review the resident's EMR for treatments, medication orders, diets, etc.
• Make periodic checks to ensure prescribed treatments are being properly administered by CNAs and to evaluate the resident's physical and emotional status.
• Notify the resident's physician and P.O.A./Guardian of changes in condition.
• Administer professional services such as; catheterization, tube feedings, wound care, etc.
• Ensure that personnel providing direct care to residents are providing such care in accordance with the resident's care plan and wishes

Full and Part Time positions are available:

AM (6:00am-2:30pm) PM (2:00pm-10:30pm) NOC’s (10:00pm-6:30am)

All scheduled positions require an every other weekend and every other holiday rotation.

• Consult with the resident's physician in providing the resident's care and treatment
• Review the resident's EMR for specific treatments, medication orders, diets, etc.
• Make periodic checks to ensure that prescribed treatments are being properly administered by certified nursing assistants and to evaluate the resident's physical and emotional status
• Notify the resident's physician and P.O.A./Guardian of changes in condition
• Administer professional services such as; catheterization, tube feedings, wound care, etc.

Requirements
