**Position:** Admissions Coordinator - Home Care (Full Time)

**Organization:** Your Choice At Home, an affiliate of Cobble Hill Life Center and a Certified Home Health Agency (CHHA). We operate 24/7, providing reliable, dignified care in patients' homes, specializing in health services, personal care, and champion services.

We are seeking a Licensed Nurse (RN/LPN) to serve as the primary contact for prospective patients and their families, guiding them through the admissions process with empathy, professionalism, and expertise. This role is crucial in ensuring a smooth admission process for patients into our CHHA program.

Key Responsibilities:

• Coordinate patient referral and approval processes efficiently.
• Assign initial ICD-10 codes to initiate authorization for HMO referrals.
• Build and maintain relationships with hospitals, physicians, and community organizations to generate admissions.
• Understand and adhere to policies, procedures, assessments, and paperwork requirements for admissions.
• Communicate special needs of new admissions to staff for smooth transitions.
• Maintain knowledge of Medicare/Medicaid/Managed Care reimbursement regulations.
• Conduct assessments to determine patient care needs and eligibility, collaborating with healthcare professionals.
• Develop marketing strategies to promote CHHA services and attract new patients.
• Collaborate with interdisciplinary teams to coordinate admissions and provide comprehensive patient care.

Qualifications:

• Active RN or LPN license in New York State.
• Degree from an accredited nursing program.
• Experience in healthcare admissions or discharge planning; hospital discharge planning experience is a plus.
• Knowledge of Home Care billing, including Medicare, Medicaid, and third-party payors.
• Familiarity with ICD-10, CPT-4 coding, and medical terminology.
• Excellent communication skills.
