Northshore Health Centers, Inc. is seeking a Mental Health Therapist to provide counseling for individuals, couples, and families and to coordinate behavioral health services under the direction of the Director of Behavioral Health.

You will educate patients on behavioral health issues, develop treatment plans, and collaborate with healthcare providers to ensure high-quality care. The role involves documentation in EHR systems, crisis support as needed, and active participation in program

Formación

• Associate's Degree in Counseling, Psychology, Social Work or related field.
• Licensed Social Worker or Mental Health Therapist in the State of Indiana.
• BLS certification within two weeks of hire.

Responsabilidades

• Conducts patient assessments, including behavioral health history and scales.
• Develops and coordinates personalized treatment plans and goals.
• Communicates with primary care providers about treatment options.
• Provides individual and group therapy using evidence-based methods.
• Maintains detailed records in the electronic health record.
