Job summary

Lead patient-centered care by developing treatment plans, coordinating resources, and advocating for patients and families. Candidates should have MSW from CSWE-accredited programs and an AZ LMSW license, with strong crisis intervention and collaboration skills.

Qualifications

• One (1) year Post-MSW experience or Social Work internship in clinical/medical setting
• LMSW: AZ licensure
• MSW from CSWE-accredited school
• Ability to work independently with attention to detail
• Ability to work in a fast-paced environment with changing priorities
• Ability to collaborate with an interdisciplinary care team

Responsibilities

• Perform comprehensive psychosocial assessments and develop treatment plans incorporating medical, social, and resource issues
• Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families
• Serve on hospital committees and present in-services to hospital staff
• Participate in performance improvement programs as needed
• Model the organization’s Mission and Core Values in daily work
