Job summary

Mission Healthcare is looking for a Licensed Medical Social Worker in Phoenix, AZ, to conduct psychosocial assessments and create tailored intervention plans for patients. Ideal candidates possess a Master's degree in Social Work and must have an active clinical license along with at least one year of healthcare experience.

This is a per diem position with a schedule of Monday to Friday, 8 AM to 5 PM. Employees have access to valuable benefits, including a 401(k) plan, EAP, and tuition

Qualifications

• Active license as a clinical Social Worker by the Board of Behavioral Sciences.
• Minimum of 1 year of experience in healthcare, preferably home health care.
• Current BLS, hands-on CPR certification.

Responsibilities

• Conduct psychosocial assessments of patients.
• Develop and implement tailored intervention plans.
• Maintain accurate clinical records for all patients.
