OSUP Community Medical Group is seeking a Social Work Case Manager in Ohio to identify at-risk patients, assess needs, and coordinate interventions with the care team.

You will link patients to community resources, advocate for services, and educate staff on referrals and available programs.

Qualifications: BS in Social Work (MSW preferred), Ohio LSW licensure, healthcare experience, and strong written and verbal communication.

Qualifications

• Bachelor’s Degree in Social Work required; Master’s degree preferred.
• Ohio LSW licensure is required or must be obtained.
• Experience in a healthcare environment is preferred.
• Experience coordinating community resources and payer programs is required.
• Strong written and verbal communication skills are essential.

Responsibilities

• Identify at-risk populations and assess needs.
• Provide case management and coordinate interventions.
• Link patients with community resources and ensure access to services.
