Job summary

AVIR Health Group is seeking a qualified MDS Coordinator to join our family. We are searching for a licensed MDS Coordinator who is resident & family focused, a team builder, and excited about the opportunity to build and lead a high-performing team and culture.

The primary purpose is to complete the RAI process for each resident of the facility, as required by State and Federal regulations. This includes transmission and verification of receipt by the State.

Walk-ins are welcome daily!

Qualifications

• Valid Texas Nursing License.
• CPR Certification is required.
• Minimum of one year of supervisory experience in a hospital, skilled nursing facility, or related healthcare program.
• Knowledge of long-term care standards per Federal Register, Conditions of Participation and State regulations.

Responsibilities

• Complete the RAI process for each resident as required by State and Federal regulations.
• Transmit and verify receipt of RAI data by the State.
• Lead and develop a high-performing MDS team and culture.
