Job summary

Mercy Gilbert Medical Center is seeking a Licensed Medical Social Worker to perform psychosocial assessments, develop treatment plans, and coordinate resources for discharges across inpatient, outpatient, and Emergency Department settings.

You will advocate for patients and families, address social determinants of health, and participate in performance improvement initiatives while upholding compassionate, patient- and family-centered care.

Qualifications

• 1 Year Post-MSW experience or Social Work internship in a clinical or medical setting
• LMSW license from the AZ Board of Behavioral Health Examiners
• Master's Degree from a School of Social Work accredited by the Council on Social Work Education
• Ability to work independently with a high degree orientation
• Ability to work in a fast-paced environment with changing priorities
• Ability to collaborate with an interdisciplinary care team

Responsibilities

• Perform comprehensive assessments and develop treatment plans integrating medical, social, and resource issues affecting patients and families
• Establish professional relationships with physicians, case managers, nursing staff, county agencies, and community resources
• Serve on hospital committees and develop in-services for hospital staff
• Participate in performance improvement teams as necessary
• Demonstrate alignment with the organization's mission and core values

Job Summary and Responsibilities

As our Licensed Medical Social Worker at Mercy Gilbert Medical Center, you will perform critical social work assessments and interventions for inpatient, outpatient, and Emergency Department patients. You'll lead patient-centered care by developing and implementing treatment plans, facilitating post-acute placements, and coordinating comprehensive resources for complex discharges and vulnerable populations. This role is central to crisis intervention, patient advocacy, and ensuring developmentally appropriate care across all served populations, from mentally ill to chronically ill.

Every day you willmanage psychosocial care, including crisis intervention, counseling, and referrals for diverse issues like abuse, family challenges, and grief. You'll serve as a patient advocate, educating on self-care, bioethics, and insurance, while actively coordinating community resources to address social determinants of health such as homelessness. You'll also contribute to performance improvement teams, ensuring adherence to federal, state, and regulatory guidelines to enhance financial outcomes and reduce readmissions.

Your ability to independently manage multiple priorities, work efficiently in a fast-paced environment with changing demands, and collaborate effectively within an interdisciplinary team is crucial.
