City on a Hill Inc. is hiring a Licensed Master Social Worker to support urban families navigating trauma in a free clinic setting. Purpose and Scope
City on a Hill Inc. is seeking a dedicated and compassionate Licensed Master Social Worker to join our multidisciplinary team within our free and charitable clinic setting. The ideal candidate will work collaboratively to develop and implement comprehensive care plans that integrate medical, behavioral health, and dental services. This role focuses on serving urban families of color who are navigating high levels of violence, crime, and trauma. The Social Worker will function within a whole-family case management approach, providing critical support to both clients and staff to promote positive outcomes and improved quality of life.
Key Responsibilities
1. Client Assessment and Care Planning
• Conduct comprehensive psychosocial assessments to identify client strengths, needs, barriers, and goals.
• Develop individualized, culturally sensitive care plans in collaboration with clients, families, and the interdisciplinary team.
• Utilize evidence-based practices to address trauma, mental health issues, and social determinants of health.
2. Case Management and Counseling
• Provide individual and group counseling services focusing on trauma-informed care, crisis intervention, and psychosocial support.
• Assist clients in accessing community resources such as housing, employment, education, and legal services.
• Advocate for clients within healthcare systems and community agencies to ensure comprehensive support.
3. Collaboration with Maximus Team
• Work closely with Maximus’s Frontline Employment Planners (FEPs) to consult on developing effective employment plans.
• Provide expertise on psychosocial factors affecting employment and family dynamics.
• Participate in interdisciplinary team meetings with Maximus staff to coordinate care and services.
4. Staff Support and Education
• Offer training and consultation to Maximus and COAH staff on psychosocial issues, trauma-informed care, and cultural competencies.
• Support staff in understanding the impact of community violence, trauma, and systemic barriers on clients.
5. Documentation and Compliance
• Maintain accurate and timely documentation of all client interactions, assessments, care plans, and progress notes.
• Ensure compliance with organizational policies, state regulations, and confidentiality laws (e.g., HIPAA).
• Participate in quality improvement activities and utilize data to inform practice.
6. Community Outreach and Resource Development
• Build and maintain relationships with community organizations, service providers, and support networks.
• Stay informed about local resources and programs to enhance client support.
• Represent COAH at community events, meetings, and collaborative initiatives.
7. Professional Development
• Engage in ongoing training and education to remain current with best practices in social work and mental health.
• Participate in supervision and professional development opportunities provided by COAH and Maximus.
Qualifications
Education and Licensure
• Master’s Degree in Social Work (MSW) from an accredited institution.
• Current and valid license as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of Wisconsin.
Experience
• Minimum of 2 years of experience working with urban populations, particularly families of color affected by high violence, crime, and trauma.
• Experience in a clinical setting, preferably within a free or charitable clinic or community-based organization.
• Familiarity with employment services, workforce development, or collaborating with employment-focused agencies is a plus.
Skills and Competencies
• Strong understanding of trauma-informed care, mental health interventions, and the social determinants of health.
• Excellent assessment, counseling, and case management skills.
• Ability to work effectively within a multidisciplinary team and collaborate across organizations.
• Cultural competency and sensitivity to the unique needs of diverse populations.
• Proficiency in electronic health records and documentation systems.
• Strong organizational, communication, and interpersonal skills.
• Bilingual abilities (e.g., Spanish, Hmong) are highly desirable but not required.
Physical Demands and Working Conditions

• Flexibility to accommodate clients’ needs, which may include occasional evenings or weekends.
• Local travel within the Milwaukee West Central region (zip codes: 53208, 53216, etc.) for client meetings and community outreach.

The Social Worker will function as part of a collaborative effort between COAH and Maximus to support the whole-family case management approach, enhancing services for Wisconsin Works (W2) participants.
