As an LMSW, you'll play a vital role in supporting patients facing mental health challenges, addiction, and crises, helping them find stability and improved well‑being.

What You'll Do

• Conduct in‑depth mental health assessments to identify patient needs.
• Develop, implement, and evaluate individualized treatment plans.
• Provide outpatient therapy tailored to adults and/or children with diverse mental health needs.
• Offer crisis intervention services, ensuring immediate support and patient safety.

Collaboration & Community Support

• Work closely with interdisciplinary teams to provide holistic patient care.
• Establish and maintain relationships with community organizations for patient advocacy.
• Connect individuals with mental health and public health resources to enhance their support network.

Monitoring & Reporting

• Maintain accurate patient records, treatment plans, and progress reports.
• Conduct regular assessments to evaluate patient improvement and adjust treatment strategies.
• Prepare detailed case summaries and discharge plans.

What You Bring

Required Qualifications

Education: Masters Degree in Social Work (MSW).

License: Active LMSW licensure.

Experience: At least 3 years of experience working with diverse populations, including expertise in:

• Conducting mental health assessments and developing treatment plans.
• Providing individual and family therapy.
• Crisis intervention and behavioral health support.
• Proficiency in evidence‑based practices and psychosocial assessment.
• Strong treatment planning skills with the ability to adapt plans as needed.
• Excellent communication skills for patient counseling and interdisciplinary collaboration.
• Ability to handle crisis stabilization and make emergency decisions.
• Familiarity with local mental health resources for patient referrals and advocacy.
