Job summary

The New York State Office of Mental Health is recruiting a Licensed Master Social Worker at Central New York Psychiatric Center, Attica Satellite Unit. You will contribute to a multidisciplinary team, delivering evidence-based social work services to individuals with mental illness in a recovery-oriented setting.

Responsibilities include crisis intervention, discharge planning, developing and monitoring treatment plans, and coordinating with community providers to ensure continuity of care.

Qualifications

• Must be LMSW licensed and registered by NYS, or eligible under NY HELPS program.
• Within State Service: one year of permanent competitive or 55-b/55-c service in title eligible for transfer.
• Background checks will be required.

Responsibilities

• Providing crisis intervention, service linkages, and discharge planning
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
• Determining discharge needs and appropriate community resources
• Establishing and maintaining relationships with community providers to ensure continuity of care
