Job summary

Valley Hospital Phoenix is seeking a dynamic and talented Intake Therapist (MA/MSW) to join our team of compassionate, dedicated professionals.

The Licensed Therapist performs social services and psychotherapy to assist patients in meeting their biopsychosocial needs while providing assessment, diagnosis, and psychotherapy treatment of individuals, couples, families, and groups to assist patients in achieving their optimal level of emotional health, including providing basic consultation,

Qualifications

• Master’s degree from an accredited college or university in social work, mental health, or related degree.
• A minimum of three (3) years’ experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.
• Licensure: LMSW, LCSW, LAC, LPC, LIAC, LMFT, LAAC, LAMFT, or Licensed Psychologist required or the ability to obtain within 3 months of hire.

Responsibilities

• Communicate with the patient’s treatment team and have a thorough understanding of the patient’s behaviors, current presentation, and future treatment goals
• Coordinate care with family, community, outpatient providers, and inpatient team via meetings with the individual responsible physician
• Carry a caseload, meet with patients daily, and maintain accurate document

Education

Master’s degree in social work or related field
