Job summary

Montage Health seeks an Ohana Licensed Intake Clinician to support children and adolescents and their families through the admission process, coordinating referrals and intake assessments across care levels. You will liaise with hospital departments and external partners to ensure smooth transitions and follow-up, participating in treatment team rounds with therapists and psychiatrists.

Required are a Master’s degree in a clinical field and California LCSW/LMFT licensure, plus 3+ years’

Qualifications

• Must have a minimum of 3 years' experience working with children, adolescents, or families.
• Licensure with the California Board of Behavioral Sciences as an LCSW or LMFT is required.
• Master’s degree in Clinical Social Work, Clinical Counseling, Clinical Psychology or Master’s Degree in Marriage and Family Therapy is required.

Responsibilities

• Support patient access across all levels of care from first contact to entry into care.
• Coordinate referrals and intake assessments, ensuring timely follow-up.
• Maintain relationships with referral partners and external providers for continuity of care.
• Attend treatment team meetings and collaborate with therapists, psychiatrists, and other behavioral health staff.
