Community Memorial Health System is seeking a licensed clinical social worker (LCSW) with an MSW to provide psychotherapeutic services and case management in an ambulatory care setting in Ventura, CA. The role involves assessment, diagnosis, and developing treatment plans, with progress monitored through sessions and calls.

The successful candidate will join an interdisciplinary team, coordinate resources, and ensure high-quality patient care in a compassionate environment.

Qualifications

• MSW degree is required.
• Active, unrestricted LCSW licensure required.

Compensation

Salary Range: $45.37 - $66.91 / hour

The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range. Responsibilities

Position Overview:

Provides psychotherapeutic services, case management, and support for patients in an ambulatory care setting in person and/or telephonically. Provides assessment and diagnosis for psychological, emotional or behavioral disorders. Makes recommendations and implements appropriate treatments plans. Monitors progress through follow-up sessions and/or phone consultations. Serves as a member of interdisciplinary team consulting with physicians, APPs, nurses, psychiatrists, and medical staff. Assures the deliverance of quality treatment to patients utilizing best practices and standards of care. Provides appropriate coordination of resources, referrals, and support in an effort to meet the total needs of the patient.

Qualifications

Required:

• Master's Degree in Social Work (MSW)
• Active, unrestricted Licensed Clinical Social Worker (LCSW) licensure

Preferred:

• Previous experience in the continuity of patient care planning
• Previous experience with crisis intervention
• Bilingual in Spanish
