Job summary

Interim HealthCare Inc. in Tucson, Arizona is seeking a Home Health Licensed Clinical Social Worker (LCSW) to deliver compassionate, home-based social work services.

You will join a team providing patient-centered care, coordinate with a home health team, and support patients and families toward meaningful outcomes. The role requires an MSW and active AZ LCSW, with at least one year of social work experience (preferably in home health), CPR certification, and a valid AZ fingerprint card.

Qualifications

• Master's degree in Social Work (MSW) and active LCSW license in Arizona.
• Minimum of one year of social work experience, ideally in home healthcare.
• CPR Certification (demonstration course required).
• Valid Arizona Fingerprint Card.
• Knowledge of state and federal home health regulations.
• Strong interpersonal skills; empathetic, compassionate, resourceful, and an effective communicator.

Responsibilities

• Provide home-based social services to patients in the home health setting.
• Collaborate with a home health team (RN, LPN, CNA, HHA, PT, OT, SLP) to support the plan of care.
• Monitor patient psychosocial condition and identify social and emotional needs.
• Conduct patient assessments, document progress, and ensure progress toward goals.
• Provide counseling, community resource planning, crisis intervention, and advocacy.
• Consult with family and caregivers on the care plan and progress.

As a Home Health Licensed Clinical Social Worker, here’s a big-picture view of what you’ll do:

• Provide home-based social services to patients dealing with depression, anxiety, poor nutrition, or lack of resources that make managing their injury or disease difficult
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT, OT, or SLP, focused on the patient’s plan of care and individual goals
• Monitor a patient’s psychosocial condition and identify social and emotional needs
