Job summary

Rehab Without Walls® Neuro Rehabilitation in New York is seeking a skilled Licensed Clinical Social Worker (LCSW) to join their team. You will provide therapeutic interventions and engage patients in achieving their recovery goals while working within their community and home environments.

Qualifications

• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury or spinal cord injury.
• Current BLS (Basic Life Support) First Aid and CPR certification required.

Responsibilities

• Perform comprehensive evaluations, assessing the social and emotional support needs of patients.

Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Perform comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families.
• Set measurable objectives that are formulated in conjunction with the rehab team.
• Assist the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems.
• Instruct/counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
• Participate in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs.
• Identify and assist the patient and/or family to utilize appropriate community resources to achieve measurable objectives.

Qualifications

What you’ll need:

• Master's Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis.
• Current BLS (Basic Life Support) First Aid and CPR certification required.
• Ability to lift 50 pounds.
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times.
