True North Consultants, Inc. seeks an experienced Licensed Clinical Social Worker (LCSW) for a permanent, full-time position in rural northeast Oregon.

The role features four 10-hour days, day shift, and a demanding but rewarding caseload within a hospital’s behavioral health team. You will perform psychosocial assessments, crisis intervention, and care coordination with physicians and nursing staff to support seamless transitions between inpatient units and community resources.

Qualifications

• Master of Social Work (MSW) degree from an accredited institution.
• Current, active license as a Licensed Clinical Social Worker (LCSW) or eligible for immediate licensure in Oregon.
• 1–2 years of clinical social work experience, preferably in hospital, clinic, or integrated healthcare setting.
• Comfort with and genuine interest in living and practicing in a rural community setting.

Responsibilities

• Conduct comprehensive psychosocial assessments, mental health screenings, and diagnostic evaluations for patients and families.
• Provide evidence-based crisis intervention, short-term counseling, and medical social work services to patients with acute or chronic conditions.
• Coordinate care transitions with physicians, mid-level providers, and nursing staff between inpatient units, outpatient clinics, and community resources.
• Educate patients and families on disease pathology, coping mechanisms, and healthcare decisions.
• Identify barriers to health and safety, facilitate emergency interventions, and arrange post-discharge care including home health or equipment.
• Maintain accurate, timely documentation of patient encounters in the EHR for billing and continuity of care.
• Participate in multidisciplinary rounds to discuss cases and promote holistic patient wellness.

Key Responsibilities:

• Identify barriers to health and safety, facilitate emergency interventions when necessary, and arrange for post-discharge care, including home health, rehabilitation, or medical equipment.
• Maintain accurate, timely, and secure documentation of all patient encounters within the electronic health record system for billing and strict continuity of care.
• Participate actively in multidisciplinary team rounds to discuss complex cases, optimize clinical outcomes, and champion holistic patient wellness.

Qualifications:

• Master of Social Work (MSW) degree from an accredited institution
• Current, active license as a Licensed Clinical Social Worker (LCSW) or eligible for immediate licensure in the state of Oregon
• 1–2 years of clinical social work experience, preferably within a hospital, clinic, or integrated healthcare setting
• Comfort with and a genuine interest in living and practicing within a rural community setting
• Excellent crisis intervention, communication, and interpersonal skills with a strong collaborative mindset
