The position features four 10-hour days, day shift, and a strong emphasis on collaborative care in a rural hospital setting. You will perform psychosocial assessments, crisis intervention, care coordination with medical staff, and post-discharge planning, with a competitive compensation up to $88,000 plus bonuses and generous CME, licensure support, and a 401(k)

Qualificações

• MSW degree from an accredited institution.
• Current, active license as an LCSW or eligible for licensure in Oregon.
• 1–2 years of clinical social work experience in hospital, clinic, or integrated healthcare setting.
• Willingness to live and practice in a rural community.

Responsabilidades

• Conduct psychosocial assessments, mental health screenings, and diagnostic evaluations.
• Provide crisis intervention, short-term counseling, and medical social work services.
• Collaborate with physicians, mid-level providers, and nursing staff to coordinate seamless care transitions.
• Educate patients and families on disease pathology, coping mechanisms, and healthcare decisions.
• Identify barriers to health, arrange post-discharge care including home health or rehab.
• Maintain accurate documentation of encounters in the EHR for billing and continuity of care.
• Participate in multidisciplinary rounds to discuss complex cases and improve outcomes.

A premier hospital in beautiful Northeast Oregon is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join their expanding behavioral healthcare staff. Operating on a highly desirable schedule of four 10-hour days, this role offers an exceptional work-life balance in a supportive, collaborative environment. Key Responsibilities:

• Conduct comprehensive psychosocial assessments, mental health screenings, and diagnostic evaluations for patients and families.
• Provide evidence-based crisis intervention, short-term counseling, and medical social work services to support patients dealing with acute or chronic medical conditions.
• Partner directly with physicians, mid-level providers, and nursing staff to coordinate seamless care transitions between inpatient units, outpatient clinics, and community-based resources.
• Support, counsel, and educate patients and their families on disease pathology, coping mechanisms, and complex healthcare decisions.
• Identify barriers to health and safety, facilitate emergency interventions when necessary, and arrange for post-discharge care, including home health, rehabilitation, or medical equipment.
• Maintain accurate, timely, and secure documentation of all patient encounters within the electronic health record system for billing and strict continuity of care.
• Participate actively in multidisciplinary team rounds to discuss complex cases, optimize clinical outcomes, and champion holistic patient wellness.

Qualifications:

• Master of Social Work (MSW) degree from an accredited institution
• Current, active license as a Licensed Clinical Social Worker (LCSW) or eligible for immediate licensure in the state of Oregon
• 1–2 years of clinical social work experience, preferably within a hospital, clinic, or integrated healthcare setting
• Comfort with and a genuine interest in living and practicing within a rural community setting
• Excellent crisis intervention, communication, and interpersonal skills with a strong collaborative mindset
