Qualifications

Responsibilities

• Provides comprehensive psychosocial assessments and develops treatment plans.
• Delivers behavioral health interventions and education to patients and families.
• Documents reports in a timely fashion per policy.
• Provides services in accordance with licensure regulations.
• Assesses social determinants of health and provides referrals as needed.
• Collaborates with patient and family to develop discharge plans.
