Job summary

Responsibilities

• Biopsychosocial assessments for patients across settings.
• Coordinate services to home care and equipment post-discharge.
• Participate in care conferences and referrals to rehab/long-term care.
• Provide individual and family counseling; support in ED and hospital.
• Navigate Department of Children and Families (DCF) processes.
