Job summary

DAP Health in Sunrise - Palm Springs, CA seeks an Licensed Clinical Social Worker (LCSW) to join its outpatient behavioral health team within a Federally Qualified Health Center. The LCSW will conduct triage, assessments, and psychotherapy services for diverse patients.

Responsibilities include creating treatment plans, coordinating care with multidisciplinary staff, and providing therapy via in-person or telehealth modalities while ensuring HIPAA-compliant documentation and patient privacy.

Qualifications

• Active and unrestricted California LCSW licensure.
• BLS certification through the American Heart Association or American Red Cross.
• Experience providing psychotherapy in a clinical setting.
• Experience with co-occurring mental health and substance use disorders.
• Experience in FQHC settings preferred.
• Bilingual English/Spanish or English/Arabic preferred.

Responsibilities

• Conduct comprehensive behavioral and mental health assessments, including formulating diagnoses and providing treatment recommendations.
• Develop individualized treatment plans, monitor their progress, and adapt them as needed in collaboration with the patient.
• Provide brief individual, family and/or group therapy as needed to patients with behavioral health and co-occurring disorders.
• Offer interventions and resources to patients in a mental health crisis, and coordinate referrals to higher levels of care when needed.
• Work as part of a multidisciplinary team in collaboration with other care team members including doctors, clinicians, nurses, medical assistants, case workers, education professionals, etc.
• Deliver services through a range of modalities, such as in-person, telehealth, telephone, video that are clinically appropriate and tailored to the patient's needs.
• Complete all documentation in a timely manner and protect patient privacy following HIPAA standards.
• Attend and participate in scheduled meetings and supervise trainees and interns as assigned.

Job Summary

The Licensed Clinical Social Worker (LCSW) will be an integral member of DAP Health's integrative care team, responsible for conducting triage, performing comprehensiveassessments, and providing psychotherapy services to patients in need of behavioral health care. This position operates within an outpatient Federally Qualified Health Center (FQHC) setting, where the LCSW will collaborate closely with providers and care teams, working with patients from diverse backgrounds and varying needs.

Supervisory Responsibilities: None

Essential Duties/Responsibilities

• Conduct comprehensive behavioral and mental health assessments, including formulating diagnoses and providing treatment recommendations
• Develop individualized treatment plans, monitor their progress, and adapt them as needed in collaboration with the patient
• Provide brief individual, family and/or group therapy as needed to patients with behavioral health and co-occurring disorders
• Offer interventions and resources to patients in a mental health crisis, and coordinate referrals to higher levels of care when needed
• When appropriate, refer patients to additional services such as psychiatric evaluation, substance abuse evaluation, case management, intensive outpatient programs,hospitalization etc.
• Work as part of a multidisciplinary team in collaboration with other care team members including doctors, clinicians, nurses, medical assistants, case workers, education professionals, etc.
• Act as a liaison and coordinate care between patient and community resources as needed
• Deliver services through a range of modalities, such as in-person, telehealth, telephone, video that are clinically appropriate and tailored to the patient's needs
• Complete all documentation in a timely manner
• Protect patient privacy following HIPAA standards
• Attend and participate in scheduled meetings
• Provide supervision of trainees and interns as assigned
• Perform additional duties as requested

QualificationsRequired Skills/Abilities

• Knowledge and expertise in utilizing various psychotherapy practices and methods, including evidence-based approaches such as cognitive-behavioral, interpersonal, dialectical behavior therapy, and more
• Ability to stay calm and supportive during psychological emergencies or crises, providing appropriate interventions and resources as necessary
• Ability to work collaboratively within an interdisciplinary medical team as well as independently
• Awareness and sensitivity to diverse multicultural communities
• Excellent oral and written communication skills
• Effective interpersonal skills

Education and Experience

• Active and unrestricted California Licensed Clinical Social Worker (LCSW)
• Current BLS certification obtained through the American Heart Association or American Red Cross
• Experience:
• Using Electronic Health Record (EHR)
• Providing psychotherapy treatment
• Working with individuals with co-occurring mental health and substance use disorders
• Working with underserved populations preferred
• Working in FQHC preferred
• Bilingual (English/Spanish, English/Arabic) preferred
• Minimum of two years of experience providing psychotherapy in a clinical setting as an ASW and/or LCSW with experience working with children, adolescents, and adults preferred

Working Conditions/Physical Requirements

• This position is based on-site at a DAP Health clinic, with the option for hybrid or fully remote work, along with occasional travel to DAP Health locations
• Operates in an office/clinical setting and requires frequent times of sitting, standing, repetitive motion, and talking
• Requires current and valid driver’s license and current personal auto insurance
• Able to travel to DAP Health locations throughout San Diego and Riverside for patient care and meetings
• Ability to lift 24 pounds
