Job summary

Sweetwater Care, a Massachusetts-based skilled nursing facilitator, seeks a Regional Licensed Clinical Social Worker LCSW to join our Boston-area team. You will provide assessments, therapy, and case management while overseeing the workflow and outcomes of other LCSWs and social service staff.

Applicants must hold a MA LCSW, have at least 2 years of clinical experience, and possess a relevant Bachelor’s or Master’s degree.

Qualifications

• Must hold LCSW license in MA.
• Minimum 2 years of clinical social work experience.
• Bachelor's or Master's degree in Social Work or related field.
• Strong communication and interpersonal skills.
• Ability to work in fast-paced environment and manage priorities.
• Active NPI in good standing with the state.

Responsibilities

• Provide clinical services including assessments, individual and group therapy, and case management.
• Develop and implement individualized treatment plans in collaboration with clients and their families.
• Conduct mental health evaluations, crisis intervention, and referrals to community resources.
• Collaborate with multidisciplinary treatment teams to ensure comprehensive care.
• Maintain accurate and timely records of client services and treatment plans.
• Participate in quality improvement initiatives and contribute to the development of program policies and procedures.
• Guide and mentor other LCSWs in the same demographic.
