Are you passionate about helping frail, underserved, and multicultural senior communities live independently and thrive at home? We are looking for a Licensed Clinical Social Worker (LCSW) to join our team and provide essential behavioral health services to elderly participants across Fresno, Bakersfield, and Anaheim.

What We Offer:

• Competitive Pay: $47 - $53.00/hr, based on experience

Key Responsibilities:

• Conduct behavioral health assessments and develop care plans with the interdisciplinary team
• Offer individual and group therapy, crisis intervention, and case management
• Mentor and supervise MSW trainees working toward LCSW licensure
• Assist with intake and enrollment processes for new participants
• Provide training and support to PACE staff on behavioral health interventions

What You Bring:

• Experience: Minimum of 1 year working with frail or elderly populations, plus 4+ years providing direct services to people with chronic mental illness and substance abuse issues
• Skills: Excellent clinical, organizational, and communication skills; ability to work in a fast-paced, interdisciplinary setting
• Certifications: Valid CA LCSW, BLS/First Aid Certification, and current CA driver’s license
• Education: MSW required
