Job summary

Community Hope, Inc. is hiring a full-time Licensed Case Manager for the Hope for Veterans Program in New Jersey. You will provide assessment, individual and group psychotherapy, and supportive services to veterans enrolled in the program.

Responsibilities include direct services, recovery planning, IRPs, veteran advocacy, referrals, family meetings, and coordination with community resources. You will participate in on-call rotation and help develop recovery groups, with a 40-hour workweek.

Qualifications

• Must hold NJ LSW/LAC/LCSW or LPC licensure.
• Master's degree in a related field is required.
• Knowledge of veterans' issues, mental health, addiction, or homelessness is required.

Responsibilities

• Provide assessment, diagnosis, individual psychotherapy, group psychotherapy, and support services for veterans in the Hope for Veterans Program.
• Develop Individual Recovery Plans (IRPs) in compliance with VA Grant & Per Diem guidelines.
• Support veterans with mental health symptoms, grief, PTSD, behavioral issues, relationship challenges, or substance use.
• Participate in Clinical On-Call rotation and respond to emergencies.
• Develop recovery support groups and activities for veterans.
• Transport veterans in agency vehicles as needed.
• Provide medication education to promote safety and independence.
• Maintain accurate clinical documentation with veteran confidentiality.
• Collaborate as a team member, facilitating groups and covering co-workers' duties as needed.
