Qualifications

• Licensure required in one of the following: RN, SW, RT, PT, OT, or SLP.
• Minimum 2 years in social work or case management in inpatient settings; rehab experience preferred.
• Excellent communication, documentation, and coordination skills.

Responsibilities

• Coordinate interdisciplinary care plan and discharge planning.
• Serve as liaison among patients, caregivers, and healthcare team.
• Document care activities per CMS guidelines.
• Schedule family conferences and communicate progress.
• Coordinate weekly interdisciplinary team conferences.
• Review CMG and notify HIM of active comorbid conditions.
• Perform other duties as assigned.
