Job summary

PruittHealth is seeking a Licensed Care Manager for Home First in Georgia. The role screens referrals, coordinates admission processes, and collaborates with providers to ensure timely, quality care in the member's home.

You will travel using your own vehicle, complete in-home assessments, supervise quarterly visits, and participate in team meetings and on-call rotations to support patient health and functional status.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Valid driver's license and automobile liability insurance.
• Graduate of a state approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation (CPR).

Responsibilities

• Screen referrals and coordinate admission processes.
• Coordinate services with providers and handle billing reconciliation.
• Participate in team meetings and present new admits, annual assessments, hospitalizations, and discharges.
• Complete quarterly home visits and PCP visits with required documentation.
• Track hospital/ER visits and communicate with hospitals.
• Travel using own vehicle; arrange transportation and complete mileage sheets.
• Attend trainings; train new hires; participate in on-call rotations.
