Qualifications

• Requires bachelor's degree in social services, addiction, mental health or closely related health services field with 3+ years of post-Bachelor degree professional experience in the addictions or counseling field
• OR master's degree in social services, addictions, mental health or closely related health services field with 1+ years of post-Masters degree professional experience in the addictions, therapy or counseling field
• Degree must be from an accredited college or university.

Responsibilities

• Conduct client assessments identifying history, presentation, problem areas, and ongoing clinical need for treatment and risk assessment.
• Assess ongoing chemical dependence services required by the treatment program.
• Prepare and carry out initial treatment planning and progress reviews, with required documentation.
• Lead group, individual or family sessions and apply evidence-based treatment modalities with documentation.
• Provide client and family education on substance abuse/psychiatric problems.
• Ensure all documentation regarding client care, treatment and incidents is timely and clear.
• Provide crisis intervention and counseling as needed.
• Plan for and complete documentation of aftercare for assigned clients.
• Act as liaison with insurance carriers as requested.
• Act as liaison between referral sources and clients.
• Provide case management to ensure individualized quality care.
• Conduct educational workshops in the hospital and community as required.
• Serve as liaison with self-help groups and EAP professionals.
• Evaluate client needs to determine level of care and referrals as needed.
