A well-established organization seeks an LPN to provide nursing care under the supervision of a Registered Nurse. The role includes monitoring patient outcomes, contributing to care plans, and ensuring adherence to safety protocols. Ideal candidates should have experience in nursing homes and effective communication skills.
Shift Hours : 5x8 Evenings and Nights w / Every Other Weekend. Schedule Notes : 5x8 7 workers for - Night w / EOW - 11:00 p - 7:15 a; 8 workers for - Evening w / EOW - 3 p - 11:15 p. This location uses paper documentation only. Nursing home experience is required.
Overview : Under the direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit. The duties require tact and judgment. May assist in supervising non-professional staff. Familiar with the philosophy, standards, objectives, and policies of the Nursing Department. Performs related duties as required.
Responsibilities : Demonstrates proficient knowledge in patient care. Uses diagnostic results and other information for accurate evaluation. Applies knowledge of illnesses, injuries, and diseases, using available resources in the nursing process. Performs timely, comprehensive reevaluations and documentation, especially when significant changes occur. Respects residents' privacy during evaluations. Contributes to individualized care plans with measurable goals and updates them based on patient condition. Bases care decisions on resident needs and priorities. Monitors patient outcomes and coordinates resources for discharge planning. Collaborates with Care Managers, physicians, and other disciplines to plan interventions that meet resident and family needs efficiently.
Knowledge and Skills : Ability to work in environments with dust, noise, etc. without undue discomfort. Capable of hearing, talking, walking, standing, bending, and stretching. Able to read, write, and interpret documents. Able to sit for long periods at a desk and stand for extended times at patient bedsides or procedural areas. Effective communication with patients and co-workers. May be exposed to hazards such as mechanical, electrical, radiant, explosive, burns, and chemicals. May need to handle, dispose of, or identify hazardous waste and materials. May operate and clean equipment. Must be trained in Kingsbrook / Rutland policies regarding hazardous waste and maintain relevant qualifications.
Other : Knowledge of all codes and understanding of their application. Duties may involve exposure to blood, body fluids, and tissues, with potential for such exposure in routine tasks.
