Job summary

Delta-T Group currently seeks a Licensed Graduate Social Worker (LGSW) to support older adults in a Washington, DC long-term care facility. Schedule is 8:30am-5pm, Monday-Friday, with on-site assignments and contractor-based opportunities.

The role requires conducting intake and assessments, meeting clients per the treatment plan, and timely documentation. Collaboration with the Director and a multidisciplinary team is essential for developing and implementing plans for clients.

Qualifications

• LGSW credential required.
• Experience working with older adults in residential or community settings.

Responsibilities

• Conduct intake/assessment to determine needed services
• Meet with clients as determined by treatment plan
• Complete case notes in a timely fashion
• Work with the Director and other multi-disciplinary team members to develop and implement treatment plans/assessments for clients
