Overview
The Level 2 Post Release Services (PRS) Case Manager plays a critical role in providing essential support and services to unaccompanied children and their sponsors. This position requires a high level of technical and professional expertise to advocate for the safety and well-being of children, focusing on their successful integration into their new environment.
Responsibilities
- Provide culturally and linguistically appropriate post-placement services and case management for UAC;
- Manage a caseload of at least 15 PRS cases, with services provided virtually and in person every 90 days;
- Complete individualized needs assessments and develop case plans considering culture, language, and special circumstances;
- Offer ongoing case management based on assessed needs;
- Maintain accurate records using UAC Portal and internal systems, in compliance with policies;
- Travel up to 50% with short notice;
- Provide information, referrals, outreach, advocacy, and support;
- Assess safety and placement appropriateness via calls, interviews, and home visits;
- Ensure confidentiality per HIPAA and PHI standards;
- Coordinate communication with stakeholders;
- Participate in training sessions via Teams, Zoom, etc.;
- Maintain professional conduct and respectful interactions;
- Comply with all applicable laws, regulations, policies, and procedures.
Position Requirements
- Bachelor’s degree in social work, behavioral science, or social services with at least two years’ experience;
- Experience in child welfare, placements, or family preservation preferred;
- Strong skills in assessment report writing;
- Knowledge of local immigrant community resources advantageous;
- Excellent organizational, communication, and interpersonal skills;
- Valid driver’s license, reliable transportation, and a clean record;
- Suitable remote workspace and proficiency in Microsoft Office, especially Excel and Teams;
- Fluency in Spanish preferred; additional languages a plus;
- Commitment to child abuse and neglect reporting requirements;
- Ability to work collaboratively in a multidisciplinary team;
- Possession or ability to obtain CPR and first aid certification;
- Supportive of a respectful, multicultural workplace.
Physical Demands
- Manual dexterity, visual, tactile, and auditory acuity;
- Prolonged sitting, standing, and repetitive motions;
- Occasional lifting up to 25 pounds;
- Basic math skills for measurements and calculations.
