Job summary

Roswell Park Comprehensive Cancer Center Oncology Nurse Coordinator – Leukemia is a regular full-time role focused on care coordination, patient advocacy, and delivering an exceptional patient experience throughout the cancer care continuum.

The nurse will coordinate multidisciplinary teams, assess and implement individualized care plans, educate patients, and support navigation via phone, video visits, and the patient portal while upholding high nursing standards.

Qualifications

• Active RN license in New York.
• BLS certification or eligibility for certification.
• Bachelor’s degree in Nursing with 1 year oncology/medical-surgical/critical care experience.
• Three years of full-time medical-surgical experience in an acute care setting with a Bachelor’s in Nursing.
• Three years of full-time oncology/medical-surgical/critical care nursing with an Associate’s degree.
• Five years of full-time medical-surgical experience with an Associate’s degree.
• Incumbent must obtain and maintain applicable certification within 30 months of appointment.
• Incumbent must obtain BLS certification within 6 months of appointment.
• Complete twelve cancer-related contact hours per calendar year unless ONCC certified.

Responsibilities

• Coordinate multidisciplinary care and ensure effective communication among providers.
• Utilize the nursing process to assess, develop, implement, and evaluate individualized care plans.
• Act as the central point of contact for patient and caregiver inquiries related to care plans.
• Proactively anticipate patient needs to ensure timely navigation.
• Assess learning needs and provide education to promote adherence to established care plans.
• Enhance patient experience by delivering excellent customer service and promptly addressing concerns.
• Interact with patients via phone, virtual visits, and the patient portal to ensure care is well coordinated.
