Job summary

Guardian Angel Senior Services is seeking a compassionate Home Care RN to deliver individualized in-home nursing care to seniors in Worcester, MA and surrounding towns.

You will assess client needs, develop care plans, monitor progress, and educate clients and families while coordinating with caregivers and health professionals. Travel to multiple client sites is required in the Worcester service area.

Qualifications

• Massachusetts RN license required.
• Current CPR certification required.
• Home care experience preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Compassionate and patient-centered approach to care.
• Strong attention to detail.
• Ability to work independently and manage multiple client needs.
• Reliable transportation and willingness to travel throughout the Worcester service area.

Responsibilities

• Perform in-home clinical assessments and evaluate client needs.
• Develop and update individualized care plans.
• Monitor client progress and revise care plans as needed.
• Educate clients and families regarding care plans, treatment, and health maintenance.
• Observe caregivers in the field to ensure they are following the client's care plan and providing appropriate care.
• Provide caregiver education and re-education when needed.
• Communicate with clients, families, caregivers, and other healthcare professionals.
• Accurately document visits, assessments, and client progress.
• Maintain compliance with applicable state and federal regulations.
• Help identify opportunities to improve the quality and safety of client care.

What You'll Do as a Home Care RN

What We're Looking For
