Job summary

Community Clinic Inc. in Silver Spring, MD seeks a Lead Triage Registered Nurse (RN II) to lead the triage team, provide evidence-based nursing advice over the phone and via EMR, and coordinate transitions of care.

As a member of the integrated care team, you will supervise triage staff, develop and enforce clinical guidelines, ensure timely charting, and maintain HIPAA-compliant, patient-centered communication. This is a regular full-time on-site role.

Qualifications

• Active RN license issued by the Maryland Board of Nursing or NP/compact state license.
• Minimum of three (3) years of nursing experience, with acute care.
• Current BLS/CPR certification required.
• Experience documenting in an EMR; knowledge of eClinicalWorks (eCW) is a plus.
• Written and verbal fluency in English; bilingual a plus.

Responsibilities

• Provide nursing triage services via telephone or web consult using evidence-based resources.
• Educate patients and provide medical advice and preventative guidance.
• Lead transitions of care outreach after hospital discharge or ED visits.
• Supervise triage nursing staff including assignments, coaching, and corrective action.
• Maintain HIPAA compliant, patient-centered communication and accurate EMR charting.
