Requires a Master’s in Social Work and at least three years of medical or psychiatric social work, including one year as Social Worker II. Licensure (LMSW/LCSW) may be required in some departments.

Formación

• Master's Degree in Social Work is required.
• 3 years experience in medical and/or psychiatric social case work, including 1 year as Social Worker II.
• Licenses and Certifications: LMSW and/or LCSW may be required in some departments.

Responsabilidades

• Leads and integrates social work functions and patient‑desirable outcomes into patient care and discharge planning with external case managers, community agencies, and/or health care facilities.
• Identifies, develops, and implements programs and services to address psychological and social needs of patients and their families (e.g., support groups, educational materials, interdisciplinary inservices).
• Assesses social service needs, provides counseling, and coordinates effective individual social work treatment plans for patients and families with multiple/complex needs.
• Plans requirements for ongoing development of new or enhanced outreach services with physicians and community outreach professionals.
• Educates families and patients regarding community resources and pertinent health care issues to ensure appropriate use of resources.
• Provides discharge planning referrals and supports continuity of care for patients.
