Job summary

PACE in Eastpointe, Michigan, is seeking a Master of Social Work (MSW) to work independently within the IDT to assess needs and coordinate social services for participants and families. The MSW will conduct comprehensive bio-psychosocial assessments, develop treatment plans, and provide case management support using CBT, Motivational Interviewing, and psychoeducation.

Qualifications

• MSW from an accredited school of social work.
• Licensed to practice in Michigan (Limited license may be considered).
• One year of clinical social work experience, preferably in geriatrics.
• Strong case management, interventions, and risk assessment skills.
• Proficient in electronic health records and patient-centered care.

Responsibilities

• Perform bio-psychosocial assessments at enrollment and at 180-day intervals.
• Develop and implement treatment plans for participants and caregivers; include CBT and Motivational Interviewing.
• Coordinate services with the care team; manage crisis and safety planning as needed.
• Document accurately in the EHR and communicate changes to the IDT.
• Advocate for participants and caregivers; support discharge planning and follow-up.
