Expected pay for this position is $33.85 - $39.83/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.

Overview
POSITION SUMMARY:

The Lead Social Worker CHOI Medical Group promotes the achievement of optimal clinical and resource outcome by, assessing, planning, implementing, coordinating, monitoring interventions and develops a comprehensive plan of care for the patients. Performs ongoing communication and works to promote quality of care through collaboration with the Interdisciplinary Care Team. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The Social Worker/Program Coordinator assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with population management and transition plans. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Works with the CHOI MG leadership team to ensure alignment with the strategic goals to meet OSF Key Results. Responsible to provide daily support to frontline staff and entity leadership to triage barriers and concerns. The lead will serves as subject matter expert, onboard new employee, mentor, and resource for all CHOI MG Mission Partners/Social Workers/Program Coordinators.

Qualifications
REQUIRED QUALIFICATIONS:
Education:
  • Master’s degree in Social Work from a school or program accredited by the Council on Social Work Education
Experience:
  • 3 years of experience caring for patients with progressively increasing responsibility.
  • Broad knowledge of chronic illness and medical and community resources.
  • Positive, empathetic approach and ability to communicate effectively to patients/families with a variety of health issues, age groups, and socioeconomic backgrounds.
Other skills/knowledge:
  • Substantial interpersonal skills to develop and maintain cooperative working relationships with personnel from other departments.
  • Working knowledge of a clinical EMR system and Microsoft Office applications.
  • Able to work independently and efficiently with excellent time management skills.
  • Flexible work schedule – weekend/holiday/on call rotation.
  • Ability to successfully use independent judgment and make decisions and requires little supervision.
Licensure/Certifications:
  • Illinois or Michigan state licensing and certification requirements or jurisdiction(s) in which she or he practices.
  • A Social Worker with an MSW who has not obtained their license will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employee’s designated state (Illinois/Michigan) of practice.
  • Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employee’s designated state (Illinois/Michigan) of practice.
  • CPR Certification within 90 days of hire.
PREFERRED QUALIFICATIONS:
Experience:
  • Chronic, complex illnesses adult and/or pediatric care management experience.
  • Care Management Certification as appropriate for adult or pediatrics.
