Polaris Healthcare and Rehabilitation Center is seeking an experienced RN Unit Manager for our 100-bed Milford, Delaware facility. This full-time role requires managing the unit, applying the nursing process, and ensuring high-quality resident care.

The position emphasizes clinical leadership, assessment, care planning, and collaboration with the nursing team to maintain standards of care and regulatory compliance.

Qualifications

• Must be a licensed RN with a valid license.
• Familiarity with Federal, State, and local long-term care requirements.
• Two years of professional nursing experience.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate resident care
• Assess signs and symptoms indicating changes in the patient's condition
• Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
• Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards
• Revises the plan of care according to evaluation, changes in the medical plan of care, and effective/ineffective nursing interventions
• Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor, including establishing priorities
