Preferred Home Health Care & Nursing Services is seeking a Registered Nurse Lead for a Surgical ICU Stepdown unit to coordinate patient care, supervise staff, and ensure optimal clinical outcomes. The role emphasizes leading complex care plans, interdisciplinary collaboration, and maintaining evidence-based practices in a high-acuity environment.

The ideal candidate will have an RN license with a nursing degree, 2–5 years of nursing experience (5–10 years preferred) and demonstrated leadership

Qualifications

Current RN license with a diploma, associate’s, or bachelor’s degree in nursing.
2-5 years of nursing experience; 5-10 years preferred.
Leadership abilities to guide and mentor staff, with a focus on safety and quality.
Commitment to evidence-based practice, continuous learning, and patient-centered care.

Responsibilities

Lead development and implementation of complex, individualized care plans with multidisciplinary teams, patients, and families.
Facilitate seamless transitions from hospital to outpatient settings and address changing patient conditions.
Promote evidence-based practices through journal clubs, practice revisions, and knowledge sharing.
Evaluate patient and family experiences to improve satisfaction and care quality.
Serve as a clinical resource, provide direct patient care as needed, and support staff development.
