Haven of Paris is seeking an RN MDS Nurse to join our compassionate team in Paris, Illinois. You will lead MDS assessments, coordinate care plans, and support physicians, therapy teams, and department leaders to ensure resident-centered care.

The role emphasizes accurate documentation, adherence to state and federal regulations, and staff training on care planning best practices. Illinois RN license and MDS experience are required or preferred; AANAC certification is a plus.

Qualifications

• Current Illinois RN license and MDS experience are required.
• Experience coordinating MDS assessments and care plans.
• Familiarity with EMR documentation and clinical governance.

Responsibilities

• Coordinate timely and accurate MDS assessments.
• Lead interdisciplinary care plan meetings.
• Monitor EMR documentation for accuracy and compliance.
• Conduct in-services and training on care planning/documentation.
• Collaborate with physicians, therapy teams, and department leaders.
• Ensure compliance with state and federal regulations.
