Job summary

Millennium Healthcare Services, Inc. is seeking a Registered Nurse Clinical Supervisor to oversee home health patient care coordination and clinical operations.

This leadership role reports to the Clinical Director and supports nursing staff, ensuring care standards and regulatory compliance are met while promoting collaboration and excellent patient outcomes. The position requires CA RN licensure, BSN preferred, and prior supervisory experience in home health.

Qualifications

• Registered nurse with current CA licensure.
• BSN preferred.
• Two years of supervisory experience in home health or similar setting.
• CPR certification required.

Responsibilities

• Receive and assign case referrals to clinicians based on patient needs.
• Review cases, conduct conferences and record reviews to guide care.
• Participate in rotating on-call schedule to support staff.
• Review patients' diagnoses, medications, and care plans.
• Assist clinicians in establishing goals and plans of care.
• Attend case conferences to coordinate care.
• Contribute to policy development and quality assurance initiatives.
• Assist in staff development and orientation of new personnel.
