About the Role

Under the direction of the manager, plans, directs, and coordinates activities of a unit on a designated shift. Performs direct patient care as needed and acts as a clinical resource to nursing and support staff. Assesses, plans, evaluates, and documents the nursing process for the assigned unit.

Responsibilities

  1. Facilitates identification and resolution of care gaps from hospitalization to discharge and into outpatient settings. Critically assesses patient data to mitigate impending changes in condition and consults with the interprofessional team as appropriate.
  2. Leads the development and implementation of complex care plans with defined goals, collaborating with team members, patients, families, and caregivers. Ensures care plans reflect expert clinical knowledge, anticipate barriers, and incorporate input to improve patient outcomes.
  3. Champions evidence-based practices by participating in journal clubs, facilitating practice changes, recommending standards, and supporting nursing research and education initiatives.
  4. Evaluates practice standards, provides feedback to peers, and enhances safe patient care. Monitors patient and family experience, adjusting care to improve satisfaction and quality.
  5. This position is safety-sensitive; the ability to work alertly and safely is essential.

Minimum Requirements

  • Education: Nursing Diploma or Associate's Degree in Nursing
  • Experience: 2-5 years
  • Licenses & Certifications: RN license

Preferred Qualifications

  • Education: Bachelor's Degree in Nursing
  • Experience: 5-10 years
