The Team Lead Charge will assist in unit operations for a 24-bed acute general medicine unit, facilitating throughput, supporting clinical tasks, and ensuring documentation and care quality.

Role Purpose

Under the manager's direction, plans, directs, and coordinates unit activities, performs patient care, and acts as a clinical resource. Assesses, plans, evaluates, and documents nursing care.

Responsibilities

• Identify and resolve gaps in care from hospitalization through discharge and outpatient care.
• Develop and implement complex care plans in collaboration with the interprofessional team and patients/families.
• Support evidence-based practices and nursing practice advancement.
• Evaluate patient and family experience to improve satisfaction and care quality.
• This is a safety-sensitive position requiring alertness and safe work practices.

Minimum Requirements

Education

• Nursing Diploma/Associate's Degree in Nursing

Experience

• 2-5 years

Licenses & Certifications

• RN

Preferred Requirements

• Bachelor's Degree in Nursing
• 5-10 years of experience
