About The Role

A Lead Recovery Navigator at Equip manages a team of Recovery Navigators, serving as a care coordinator and leader within the department. This role involves guiding patients and families, ensuring alignment with Equip’s treatment model, and supporting team development. Responsibilities include team management, proactive outreach, service recovery, and acting as a liaison between patients and the care team.

Responsibilities

  • Manage a team of Recovery Navigators, providing onboarding, training, and coaching.
  • Support operational improvements and patient retention initiatives.
  • Ensure patient and family alignment with treatment goals.
  • Conduct outreach to maintain engagement and address barriers to care.
  • Monitor engagement metrics and resolve patient concerns.
  • Coordinate with other Equip teams to facilitate patient transitions and support.
  • Complete documentation accurately and timely.
  • Support training and mentorship for team members.

  • Ownership, accountability, and a passion for mental health and customer service.
  • Proficiency with digital tools (Slack, G-suite, Zoom, EMR systems).

Qualifications
