Job summary

Prava-Therapy is seeking a Lead Physical Therapist to serve as the clinical leader of therapy within a senior living memory care community. You will balance direct patient care with program leadership.

You’ll mentor clinicians, help develop new programs, and influence clinical operations as the therapy program grows within the community. This is a full-time role focused on quality care over volume.

Qualifications

• Must be a licensed Physical Therapist or eligible for PT licensure.
• Experience in geriatrics or memory care is valuable.
• Strong leadership and mentorship abilities.
• Ability to build relationships with residents, families, and interdisciplinary teams.

Responsibilities

• Provide high-quality, one-on-one physical therapy to residents.
• Serve as the primary therapy leader within the community.
• Build relationships with residents, families, caregivers, nursing staff, and community leadership.
• Collaborate with staff to identify residents who may benefit from therapy.
• Develop fall-prevention, mobility, wellness, and functional independence initiatives.
• Mentor other therapy team members as the program grows.
• Establish clinical standards and best practices.
• Participate in interdisciplinary care planning.
