Qualifications

• Role requires leadership of a rehabilitation program by a PT in a senior living community.
• Proven ability to mentor therapists and build a high-performing team.
• Capable of shaping and growing a comprehensive therapy program.

Responsibilities

• Lead the clinical and day-to-day operations of the rehabilitation program.
• Maintain an active clinical presence while providing leadership, mentorship, and support to the therapy team.
• Build a culture of clinical excellence, accountability, collaboration, and resident-centered care.
• Develop strong relationships with community leadership, nursing, wellness, and families.
• Orient and integrate new clinicians into Prava’s standards and community culture.
• Coach therapists in clinical reasoning, documentation, and patient engagement.
• Ensure therapy goals are meaningful and functional for residents.
• Collaborate with the interdisciplinary team to support residents across care continuum.
