Job summary

Alianza Health Partners is seeking a Primary Care Physician to join our growing value-based care network in Puerto Rico. You will lead comprehensive patient care, focusing on population health and quality outcomes while collaborating with a multidisciplinary team.

The role emphasizes thorough chronic disease management, wellness visits, and care coordination within a supportive care management structure. Puerto Rico license or ability to obtain is required.

Qualifications

• MD/DO from an accredited medical school.
• Board certification or board eligibility in Internal Medicine or Family Medicine (ABIM or ABFM).
• Active Puerto Rico medical license or ability to obtain within 90 days.
• DEA registration and ability to obtain Puerto Rico controlled substance registration.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Manage a panel of 1,200-1,500 attributed patients under value-based care contracts.
• Conduct comprehensive wellness visits, chronic disease management, and acute care encounters.
• Collaborate with care managers, behavioral health specialists, and social workers on complex patient cases.
• Participate in quality improvement initiatives and HEDIS measure performance.
• Engage with the MyChron population health platform for panel management and care gap closure.
• Mentor and supervise advanced practice providers as panel warrants.
