Job summary

CareMore Provider Leasing, PLLC is seeking a Physician, General / Primary Care to deliver comprehensive ambulatory and community-based primary care across the lifespan. You will evaluate, diagnose, and manage acute and chronic illnesses, perform preventive services, coordinate care with interdisciplinary teams, and guide evidence-based treatment plans.

This role emphasizes health maintenance visits, care coordination for complex patients, and mentoring APPs when needed.

Qualifications

• Licensed physician with primary care training and board-eligible status in family medicine or internal medicine.
• Experience in ambulatory and community-based care settings is preferred.
• Ability to perform office-based procedures and coordinate care with APPs.

Responsibilities

• Evaluate, diagnose, and manage acute and chronic illnesses in primary care.
• Develop and implement individualized treatment plans with preventive services.
• Coordinate care across specialists, nurses, pharmacists, and social workers.
• Lead quality improvement and ensure accurate EHR documentation.
